Book the offender. Before any retry, the transcript, tool calls, and artifacts are entered into the case file as received.
Failed Claude Code chats are remanded for examination. Five markdown slash commands preserve the record, determine the failure, impose a countermeasure, verify it, and maintain the ledger.
From the warden's desk
Inmates are held until the failure is documented and the ordered countermeasure has been verified.
Every inmate arrives with a transcript and a reported failure. The intake officer records the evidence before another attempt is made. During interrogation, the record is examined backward from the failure until the first wrong turn is found, including any error by the operator. A sentence consists of the smallest durable countermeasure that addresses the failure class. The original exchange is then re-run under the amended conditions. After verification, the inmate is released and the case file is retained in the ledger.
Intake
Five markdown slash commands for Claude Code, readable in full before any use. Case files are written to .azkaban/cases/ within the repository. Nothing is sent to azkaban.io, and the site has no access to the user's machine.
Standard procedure
Book the offender. Before any retry, the transcript, tool calls, and artifacts are entered into the case file as received.
Locate the first wrong turn. The transcript is examined backward from the reported failure until the earliest divergence is found. Errors by the operator are recorded in the same findings.
Apply the smallest fix that prevents recurrence. The sentence specifies one durable countermeasure for the identified failure class and leaves unrelated behavior unchanged.
Confirm the countermeasure fires. The original exchange is re-run under the amended conditions. A case remains open until the remedy has been verified.
Walk the cellblock. The ledger is reviewed for failure classes that recur across inmates. Each recurrence is added to the applicable case record.
Public cellblock
Members of the public may surrender a sanitized transcript for exhibition in the cellblock. Identifying particulars are struck from the record at intake, and the offender serves the remainder of its sentence in public view. Submission constitutes acknowledgment that the record joins the public ledger. Records may be removed by the warden's office without explanation.
